Individual accommodation form

The individual accommodation form (“bulletin individuel d'hébergement”) is the form every guest of a tourist accommodation fills in and signs on arrival under article 37 of law 80-14; it is the official name of the police form.

The individual accommodation form is what law 80-14 calls the police registration form. Its article 37 requires the operator of a tourist accommodation to ask every guest, on arrival, to show an ID and to fill in and sign this form, whose template is set by regulation.

What it contains

Decree 2-15-865 of 9 June 2016 sets the template in its annex. The mandatory fields are:

  • surname, first name, sex, nationality and date of birth;
  • country of residence;
  • type and number of the ID document;
  • arrival and expected departure dates, and the number of children under 18.

The operator declares these details online on the day of arrival (article 36) and keeps the forms for one year, available to the national police (DGSN) or the Royal Gendarmerie (article 38). The template can change: ask the authority that receives your declarations for the current one. Our police form guide sets out the routine, and the free generator fills in a form ready for your declaration.
